Storm Drain Dumping Complaint Form

  1. If you have witnessed anything other than rain water entering a storm drain, please use this online form to submit a report to San José’s Watershed Protection Division. The information that you provide will help us to maintain the health of our creeks and bay.

  2. If a hazardous substance is threatening human health or entering the storm drain, call 911 immediately.

  5. Please describe the incident in detail, including a description of what is entering the storm drain. Include the name of the responsible party, if known. Include make, model, color and license plate of vehicle, if applicable.

  6. Attach a photo or video that shows the problem, such as a roadway, house or structure, storm drain, creek, ditch, outfall, etc.

  7. Would you like Watershed Protection to follow up with you regarding this complaint after an Environmental Inspector has visited the site?

  9. Sometimes our Environmental Inspectors need more information about storm drain complaints, so we may follow up with you via e-mail or phone.
